How to obtain a residence permit for accompanying medical treatment in Italy

The residence permit for medical treatment allows the patient/care recipient to be assisted by a family member, relative or friend, referred to as an accompanying person.
If the accompanying person is a non-EU citizen, he or she must also apply for a specific residence permit called “residence permit for medical treatment – accompanying person.” In order to remedy the various interpretations and misunderstandings that exist on this issue, Welcome Association Italy has decided to prepare a brief information guide on the title.

To make it more functional, we conducted a survey among our members who are in Italy with this type of residence permit and summarized their experiences.

Medical care recipient and companion: a dual process

The medical care residence permit “accompagnatore” is a residence permit that allows the accompanying person to follow and support the medical treatment permit holder. The medical treatment permit holder and their companion must meet and submit different documents to legalize their stay in Italy.

Other than discussing on how to obtain a residence permit for medical treatment, we will focus on the foreigner who supports the permit holder, namely the accompanying person. The purpose of this paper is to inform about the process required to obtain a residence permit for medical treatment, as without a beneficiary of such treatment it would not be possible to have an accompanying person.

Companion health coverage

Regarding the health coverage required of the accompanying person, there are two options available, membership in the National Health System (NHS) or private health coverage. Beginning in January 2024, regulations set forth in the Budget Law 2024 went into effect. These include the significant increase in the cost of enrollment in the N.H.S. for foreign citizens. The minimum cost of membership for the foreign citizen applying for a residence permit for accompanying medical treatment is set at €2,000.00 to be combined in relation to demonstrated income. Since this is an exponential increase, it is necessary to report this information to protect foreigners in Italy.

Requirements for obtaining a medical care – companion permit

To apply for a residence permit for accompanying medical treatment in Italy, the first step is to gather basic information. Understanding Italian regulations, specific requirements and necessary documents is crucial. Documentation in this case varies slightly in that the accompanying person must present almost all of the medical care recipient’s documents with the exception of the bail payment. 1. Companion, travel preparations.

Let’s follow the path of the foreigner who travels to Italy to benefit from treatment together with his companion, a non-EU citizen. Having obtained the visa, it is time to make travel arrangements to Italy. This includes:

  • flight booking;

  • Preparation for medical needs during the trip.

It is advisable to coordinate transportation in a way that minimizes waiting time and stress for the care recipient. Ensuring that all documents, including medical and visa documents, are easily accessible is critical.

2. Arrival in Italy and initial procedures

Upon arrival in Italy, it is necessary to complete administrative procedures as soon as possible. This includes registering with local authorities and initiating paperwork for a residence permit. Registration is essential to notify the authorities of your presence in Italy and to begin the formal residence process.

3. Postal kit and appointment at police headquarters

After sending the postal kit, an appointment will be scheduled at the local police headquarters to complete registration, collect fingerprints, and provide additional required documents. This appointment is critical for verifying the identity of applicants and proceeding with the issuance of residence permits. Although the wait for the appointment and submission of documentation can be lengthy, this step is crucial to formalizing the permit application and proceeding legally with the stay in Italy.

4. Confirmation of residence permit

The residence permit will be issued later and must be picked up at the Questura. This document confirms the right to stay in Italy for the period necessary for medical treatment, both for the beneficiary and the accompanying person. Waiting times may vary, and withdrawal of the permit may take several weeks or even months. However, once obtained, it allows the applicant to remain in Italy legally and access necessary medical facilities. Possessing the permit is essential to take advantage of medical services and to regulate one’s legal status in Italy.

5. The role of the companion in Italy

During the stay in Italy, the companion plays a crucial role in providing logistical and practical support to the patient. This includes assisting with daily needs, managing medical appointments, and coordinating with the healthcare facility. Adjusting to life in Italy can take time, and the companion must be prepared for any cultural and language challenges. These may include the need to obtain an Italian tax code. The application process takes place at authorized Caf and patronati, at the Sportello Unico dell’Immigrazione, located in every Prefecture, or directly at the Internal Revenue Service.

6. Renewal of residence permit

If medical treatment requires a longer period of time than expected, it is possible to apply for the renewal of a residence permit for accompanying medical treatment. The application for renewal must be submitted sufficiently in advance of the expiration of the current permit. Italian authorities will require updated documentation on the need for treatment and proof of continued financial resources and housing. Renewal ensures the possibility of continued stay without interruption, allowing the patient to complete necessary medical treatment. Timely renewal is crucial to avoid legal problems and ensure continuity of care.

The companion, during his or her stay in Italy to support the beneficiary of medical treatment, will be able to work but will not be able to apply for conversion of the residence permit.
